The Neighborhood Health Agency (NHA) provides quality homecare and hospice services. We're here to help you take care of your patients. We also do everything we can to make the referral process fast and efficient, expedite transition from hospital to home, reduce paperwork whenever we can, provide timely, accurate, reliable information and help you and your staff in any way we can. We want you and your staff to know you can refer to NHA with confidence.

And, we are always available to provide educational/information programs for your staff in areas related to home care, hospice, family caregiver support and more.

How To Make a Referral

NHA makes it as easy as possible to make a referral to our agency.

The easiest way to make a referral to homecare or hospice is to call NHA Customer Service Center @ 1-610-696-6511 Ext. 195. You can call this number Monday through Friday, between the hours of 8 a.m. and 4:30 p.m.

You can make all necessary arrangements for services as well as medical equipment and supplies at this time.

Faxed referrals are accepted Monday through Friday, during regular business hours through our Customer Service Center fax number: 610-344-7064. Please follow up with a phone call to be sure your fax was received.

All fax referrals should include:

  • Patient's Name
  • Address
  • Date of Birth
  • Social Security Number (If Available)
  • Insurance Name and Number (If Available)
  • Type of Home Care Required
  • Frequency
  • Any Specific Orders for Care

Private insurance referrals may require pre-authorization (insurance driven) before the service can be provided. We will contact your office and advise you or your staff of any special conditions that may apply to the referral.

Initial visits to patients are usually made within 24 hours/days of referral.
