There's no place like home - especially for individuals recovering from illness or surgery or for those coping with chronic diseases and conditions. Neighborhood Health understands this and is ideally equipped to meet a wide range of patient needs. As a Medicare-Certified home care provider, Neighborhood Health's registered nurses, under the direction of the attending physician, assess, monitor, intervene with, and educate patients regarding the control of their health issues at home.
Disease and medication management using tele-health monitoring, expert wound care by Wound Certified nurses, diabetes maintenance with a Certified Diabetic Educator, and dedicated heart care with the Phase I Cardiac Program are just a sampling of the services offered by the Adult Home Care Department. Nurses, social workers, nutritionists, and home health aides, often in collaboration with rehabilitation therapists, work with patients, families, and caregivers to facilitate the understanding and care of both temporary and long-term conditions.
