At Chester County Hospital, we are committed to ensuring heart failure patients receive the most appropriate treatment according to the most up-to-date, research-based guidelines and the latest scientific evidence, with a focus on speeding recovery, improving quality of life and reducing the need for hospitalization. 

We believe that heart failure care is a team sport. Our multidisciplinary team approach to your care includes physicians, physician assistants, nurse practitioners, registered nurses, registered dieticians, nursing assistants, pharmacists, social workers and case managers, physical and occupational therapists, palliative care practitioners, and your home care team.

Our team works with you to develop a detailed self-care regimen and lifestyle management plan. We strive to build your confidence in following this regimen; we teach you to recognize and manage heart failure symptoms, eat healthy, exercise effectively and know when to call your care providers. Our team delivers this education to you and your caregivers during your hospital stay and beyond. We discuss tools you will need at home, such as bathroom scales, blood pressure monitors, pill organizers, low salt cookbooks and salt-free seasoning options.
