COVID-19 Vaccination Enrollment

CHESTER COUNTY HOSPITAL VACCINATION DISTRIBUTION INFORMATION: For Non–Affiliated Health Care Workers in 1A category. LEARN MORE! >>

This four-week educational program (one class per week) is for individuals with newly diagnosed or uncontrolled Type 2 Diabetes. Teachings review basic diabetes management principles and include the following AADE7 self-care behaviors:
  • Healthy eating
  • Being active
  • Monitoring
  • Taking medication
  • Problem solving
  • Reducing risks
  • Healthy coping

Most services are covered by insurance. All patients must have a prescription from their doctor to receive diabetes education services.

PLEASE NOTE: This will be a virtual, online presentation conducted through Zoom. After you register, Chester County Hospital will send you details and instructions so that you can connect and participate in this presentation from your home computer or other device.
